Requirements

Active Certified Nursing Assistant License
Active CPR Certification
Excellent communication and interpersonal skills
Demonstrated commitment to patient centered care
Ability to work collaboratively in a team environment

Responsibilities

Comfortable working independently performing routine nursing assistant procedures
Managing patients care within the home setting
Working in an EMR charting program
Collaborate with interdisciplinary healthcare teams to ensure coordinated patient care
