Requirements
- CNA license certification prior to hiring.
- New Graduates with their CNA license are welcome.
- Must have a current CPR and fingerprint clearance card.
- Minimum of six (6) months CNA/LNA experience preferred.
- Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience)
- LTC/SNF/Rehab preferred.
Responsibilities
- Assist guests/patients during admissions, transfers, and discharges.
- Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
- Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.
- Partner with our Therapy team to ensure they are meeting their Therapy needs on their Care Plan.
