Requirements

CNA license certification prior to hiring.
New Graduates with their CNA license are welcome.
Must have a current CPR and fingerprint clearance card.
Minimum of six (6) months CNA/LNA experience preferred.
Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience)
LTC/SNF/Rehab preferred.

Responsibilities

The CNA will provide daily care to the patients and ensure they are ready for the day with showering, capturing daily weight, taking vitals and other advanced support for our short-term rehabilitation guests.
Assist guests/patients during admissions, transfers, and discharges.
Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.
Partner with our Therapy team to ensure they are meeting their Therapy needs on their Care Plan.
