As a Bluegrass PACE Care team member, you will be a part of a program providing all-inclusive healthcare that keeps Kentuckians aged 55+, who may otherwise need nursing home care, living safely at home. Our PACE experts work collaboratively with an interdisciplinary team to decide what’s best for each PACE participant, bringing all aspects of medical and personal care, plus social and support services. Schedule: PRN (as needed) Position Summary: The PACE Home Care CNA plays a vital role in supporting the health and well-being of participants enrolled in the Program of All-Inclusive Care for the Elderly (PACE). This position is responsible for providing essential personal care and assistance with daily living activities both in the participants’ homes and within the PACE Center, as needed. The PACE Home Care CNA duties include assisting with bathing, dressing, toileting, meal preparation and feeding, monitoring vital signs, and documenting intake and output. The PACE Home Care CNA helps maintain a clean, safe, and supportive environment through routine housekeeping tasks and chore services. Additional responsibilities include transporting and assisting participants with simple errands. A CNA in this role requires compassion, attentiveness, and a strong commitment to delivering quality care while observing and reporting changes in participants’ conditions to the appropriate clinical staff.
