About The Position

Provides all necessary services needed to assist hospice patients with their
personal care and activities of daily living.

Responsibilities

• Provides direct care to hospice patients in the home. (Visit setting may be patient's physical home or SNF/ALF)
• Greets patients and their families courteously.
• Prioritizes patient care.
• Provides all identified needs of patients under the direct supervision of the Hospice RN, who operates under physician orders.
• Provides assistance with all ADLs.
• Supports the hospice philosophy.
• Provides family education while working with the interdisciplinary team.
• Provides emotional support for patients and their families.
• Provides bereavement contact to families after patient's death, with interdisciplinary team.
• Maintains a safe environment for patients and families.
• Demonstrates knowledge of emergency procedures.
• Prepares meals, snacks, and supplements as needed if requested by patients or families.
• Maintains close communication with RN supervisor regarding patient status.
• Notifies RN of any changes in patients' conditions in a timely manner.
• Performs other duties as assigned.
