ESSENTIAL DUTIES AND RESPONSIBILITIES: Provide diabetes self-management education to patients with Type 1, Type 2, and Gestational Diabetes Provide non-diabetes related Medical Nutrition Therapy services to patients from 2 yr – adult. Provide proficient assessment of health history, resource use, risk factors, cultural influence, health beliefs and behaviors, support systems, socioeconomic factors, dietary habits, and learning barriers. Educate patients in individual and group settings on basic diabetes self-management concepts, including healthy eating, physical activity, medicine administration (including instruction on injectable medications and insulin), monitoring (including glucometer and CGM instruction), problem solving, healthy coping and reducing risk of complications. Document the essential assessment, instructional content, response to interventions and the plan for follow-up in the electronic medical record. Utilizes patient data, current research and goal setting to meet educational needs of diabetes patients and their significant others. Assesses, interacts and cares for patients in a manner that demonstrates understanding of a patient’s readiness for behavior change. Utilize current educational and goal setting techniques to assist patients with behavior change and self-management. Collaborate among interdisciplinary team members in the plan of care for diabetes patients. Teach/facilitate diabetes group classes and assist in other health education opportunities within Duly Health and Care and the surrounding community. Demonstrate practices which are consistent with the professional associations (ADA, ADCES) Standards of Clinical Care. Establish relationships with representatives associated with diabetes care and nutrition to maximize knowledge base of new strategies for diabetes &/or nutrition management. Complete the mandatory hours of continuing education established by the Association of Diabetes Care and Education Specialist to maintain Diabetes Care and Education Specialist Certification and the Commission on Dietetic Registration to maintain dietitian registration. KNOWLEDGE SKILLS AND ABILITIES: Must be experienced in PC/keyboard skills and Microsoft office applications. Must possess excellent communication and writing skills. Have ability to plan, organize, and work independently. Exhibits problem-solving skills. Maintains current knowledge of up-to-date Professional Association recommendations and Insurance billing requirements for professional services rendered. Able to develop professional rapport with employees at all levels of the company. Able to work some evenings, occasional weekends. EDUCATION and / or CERTIFICATION/LICENSURE: Dietitian with a bachelor’s degree from a four-year college or university or equivalent combination of education and experience. Current certification through the Certification Board for Diabetes Care & Education CDCES OR working towards certification. Current license from the Illinois Department of Education and Registration. Current registration through the Commission on Dietetic Registration. Must have current CPR status. EXPERIENCE: Prefer at least 2000 hours of which must be in the area of direct diabetes patient education. Knowledge of insulin pumps and experience with continuous glucose monitoring devices. Knowledge of American Diabetes Association Program Recognition Standards.
