About The Position

To provide high quality healthcare through the provision of diabetes self-management training in a variety of settings to meet the needs of patients, their families and other healthcare professionals. To create and coordinate a multi-disciplinary lifestyle modification program to help participants lower their risk of heart and vascular disease through healthy exercise and eating patterns.

Requirements

  • Bachelor’s degree from an a ccredited college or university required; master’s degree preferred .
  • Completion of an A cademy of Nutrition and Dietetics (AND) approved internship program .
  • Must be a Licensed Registered Dietitian, a Registered Dietitian by the Commission on Dietetic Registration (CDR) and Certified as a Diabetes Educator by the National Certification Board of Diabetes Educators (NCBDE).
  • R ecent graduates will be considered but must be Registered and Licensed (preferred in Virginia).
  • Possesses comprehensive knowledge of and experience in diabetes prevention, prediabetes, and diabetes management.
  • Educates, supports, and advocates for people affected by diabetes, addressing the stages of diabetes throughout the lifespan.
  • Promotes self-management to achieve individualized behavioral and treatment goals that reduce risks and optimize health outcomes .
  • Ability to provide education and/or consultation for patients , family, and other healthcare professionals .
  • Ability to communicate with the healthcare team information that impacts patient care.
  • Conducts diabetes self-management education according to best practices in both individual and group setting.
  • Coordinates care, collects diabetes data, and serves as a diabetes resource to healthcare team .
  • Must be able to follow oral and written instructions and procedures and to communicate clearly.
  • Responsible for professional growth and development , functions as a clinical resource for the interdisciplinary care team , utilizes evidence-based practice to guide the provision of care . mentoring/precepting dietetic interns as assigned .
  • Participates in health fairs and other community activities that promote health , wellness , and CareAlliance Health Services .
  • Serves as a resource for compliance with modified diets through direct and indirect interaction with the diet office.
  • Patients, families and/or significant others, h ospital and medical staff , employees, and visitors.
  • Frequently standing , walking , sitting.
  • Intermittent stooping , kneeling , crouching, or crawling.
  • Intermittent lifting , moving, or carrying up to 25 lbs.
  • Frequent pushing or pulling up to 25 lbs.
  • Corrected hearing and vision to normal range.
  • Possible exposure to wet floors , steam pipes and/or variable kitchen temperatures.
  • Possible exposure to toxic or caustic chemicals , infectious materials , communicable diseases and/or other conditions common to a healthcare environment.
  • Requires work under stressful conditions , deadlines , and/or irregular hours.
  • Travel may be required.
  • Positions located in the Home Health department may have occasional exposure to communicable diseases , body fluids , unclean environments , toxic substances , pharmaceutical preparations , hazardous and unsafe driving conditions , and other conditions common to delivery of health care in the community.

Responsibilities

  • Assess es and teach es diabetes self-management education skills including blood glucose monitoring, insulin action timing and administration, medication education, symptoms and treatment of hypoglycemia and h yperglycemia, e xercise guidelines, s tress m anagement techniques, p ost-partum follow up guidelines, i nsulin pump therapy, Continuous Glucose Sensor, r eduction of risks, and s urvival skills .
  • Responsible for data collection utilizing appropriate documentation tools.
  • Participates in ADA accreditation activities.
  • Assesses, documents, and helps coordinate plan of care for appropriate measures including: HbA1c/ glucose screening tests, dilated retinal eye exams, nephropathy screening, EKG, foot exams, and patient goal setting .
  • Participates in program development as well as marketing.
  • Responsible for making outreach visits to physician offices to promote and grow diabetes related services.
  • Serve s as the nutrition and diabetes resource for the interdisciplinary team.
  • Provides education and develops resources as needed .
  • Communicates patient information to referring providers and appropriate staff in a timely manner .
  • Promote s wellness to patients, families, team members and the community by participating in health fairs, speaking engagements and other community events
