Position Purpose: The Certified Community Health Worker is responsible for serving as the initial and main point of contact between the company and the member. Supports member access to care through face-to-face visits to complete health screening and needs assessments. Certified Community Health Worker Refers member to community resources, processes reports, and distributes collateral materials. Competencies/Duties: Certified Community Health Worker coordinates follow-up care plan needs for members by scheduling appointments or enrolling members in programs. Will provide telephone and/or on onsite Recovery Support Makes referrals to the Case Management Services Assesses member compliance with medical treatment plans via telephone or through on-site visits Certified Community Health Worker identifies barriers to plan compliance and coordinates resolutions while ensuring alignment with internal company team Provide assistance accessing various community resources (education, housing, transportation, recovery supports, etc.) Recommends treatment plan modifications and determines need for additional services, in conjunction with case management and provider Certified Community Health Worker coordinates identification of and referral to local, state or federally funded programs Participates in cross-functional teams on projects, initiatives, and process improvement activities Maintains the confidentiality of all sensitive information. Accountable to understand role and how it affects utilization management benchmarks and quality outcomes
