We are currently seeking an experienced: CENTRAL INTAKE SPECIALIST - LPN Full Time Job Summary: Intake and documentation of referrals for all branches utilizing the EMR system. Intake and documentation of orders for all branches utilizing the EMR system. Promote the delivery of appropriate home health services by acting as a resource for the public. Assist in identifying resources and accurately explaining agency policies, procedures, and third party pay source coverage/reimbursement guidelines. Coordinate all services ordered. Demonstrate effectivemunication skills with all internal and external customers.

Requirements

• Requires LPN with current state licensure and successfulpletion of nursing education from an approved school of nursing.
• Must possess excellent interpersonal and oralmunication skills, especially listening and telephone skills.
• Must have broad nursing knowledge base.
• Must have working knowledge of availablemunity resources and alternatives to home care.
• Must possess considerable knowledge of home care guidelines and be able to read and interpret guidelines, policies, and procedures.
• Basicputer skills and the ability to use a keyboard.
• CPR certification andpletion of a minimum of 16 hours continuing education annually required.

Nice To Haves

• Prefer home health experience or professional nursing experiences that epass a variety of clinical settings.
