About The Position

Coordinates the overall enrollment process by assisting those interested in obtaining services for themselves and/or their loved ones. Maintains information relevant to, and works in coordination with, various agency departments, school districts, government agencies, and other providers to facilitate enrollment and manage related data. Represents the agency as a community and OPWDD liaison. Requirements

  • High School diploma or equivalent required.
  • Must possess computer proficiency in all Microsoft Suite applications, as well as excellent time management and organizational skills.
  • Valid driver’s license.

Nice To Haves

  • Bachelor's degree or some college and related experience preferred.
  • Knowledge of traditional and non-traditional OPWDD supports and services and excellent interpersonal skills preferred.

Responsibilities

  • Coordinates the overall enrollment process by assisting people with the procedures involved in seeking, transitioning, and/or changing agency supports and services.
  • Triages requests for information and supports people in the community, by directing them to the appropriate resources and informing them of available options.
  • Completes, compiles, tracks, and distributes documentation required for OPWDD eligibility, as well as program screening and enrollment, as needed.
  • Collects data and demographic information, maintaining comprehensive records for use in reporting to OPWDD, improving efficiency in enrollment, developing improved program options, and performing outreach in the community.
  • Functions as a point of contact between agency program administration, individuals and families seeking services, Care Managers, and OPWDD; tracks and initiates follow up activity.
  • Manages, investigates, and presents information based on individual screening packets and OPWDD referral lists.
  • Facilitates agency program screening, placement, and individual admissions meetings and associated tracking.
  • Maintains and provides information on all available supports and services, and remains apprised of OPWDD and other supports available.
  • Attends and facilitates Parent Orientations, District Transitional Fairs, and DORO committee meetings and disseminates information for all agency services.
  • Performs all other related duties as requested by supervisor.
