JOB SUMMARY: We are seeking compassionate and dedicated professionals to join our team as Care Coordinators in our Youth Dual Care Coordination program. This program supports children and their families in Milwaukee County who are facing challenges with severe mental health and/or substance use disorders, with co-occurring developmental and/or physical disability needs. Dual Care Coordination programming follows the wraparound model of service delivery to identify needed services and develop multi-disciplinary treatment plans that address both the strengths and needs of the family. As the Care Coordinator, you will manage a caseload of youth who are dually enrolled in Comprehensive Community Services (CCS) and Children's Long-Term Support (CLTS) programs. You will coordinate in-home and community-based services to support the families' individualized needs.

Requirements

  • Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance.
  • At minimum, a Bachelor's degree in sociology, social work, psychology, or closely related is required.

Responsibilities

  • Provide case management services to a caseload of 12-13 clients.
  • Interview and evaluate clients and families during the intake and assessment process to analyze client and family needs and strengths.
  • Create and implement goal-oriented and strengths-based treatment plans.
  • Facilitate team meetings with the client, family, and direct service providers to support and monitor progress of goals and services.
  • Develop a crisis/safety plan for the client and family.
  • Develop a family support team comprised of formal and informal supports.
  • Work in collaboration with other providers, school personnel, mental health professionals, Child and Family Services workers, and court officials.
  • Exercise judgement to intervene as an advocate for clients in emergency and crisis situations.
  • Safely transport clients as needed.
