About The Position

The following duties are normal for this position. These are not to be construed as exclusive or all‑inclusive. Other duties may be required and assigned. Ensures that the service plan and service delivery for each consumer is integrated, coordinated and monitored, and is designed to support the consumer in a manner that helps the consumer to achieve the highest level of independent functioning. Works cooperatively with other agencies specific to consumer needs with consumer consent for the purpose of continuity of care. Provide ongoing assessment and treatment planning, case management, symptom management, medication monitoring, and court monitoring of clients with serious and persistent mental illness and co-occurring substance use. Maintain accurate and timely documentation in the form of assessments, treatment plans, progress notes, etc. as required by agency policy including the authorizing and distribution of funds when warranted Serves on community based interest groups/coalitions as assigned. Assists consumers with referrals to other agencies based on individual needs. Attends and participates in agency staffing and in-service. Daily activities are performed independently. Performs duties under the clinical supervision of the Human Services Supervisor.

Requirements

  • A Master’s degree in social work preferred Bachelor’s degree may be considered.
  • Candidates with a degree in Psychology or other related field may also be considered.
  • Eligible to be certified in the State of Wisconsin required.
  • Able to converse in a direct, supportive manner.
  • Able to function under stress, to seek assistance when needed and to make decisions.
  • Able to work cooperatively with diverse agencies, families and staff members.
  • Able to preserve patient confidentiality.
  • Must have current valid Wisconsin driver’s license.
