About The Position

Esrun Health is on a mission to transform healthcare delivery from reactive, episodic care to proactively managed patient care, focusing on preventing problems for patients with two or more chronic conditions. They believe every patient with chronic disease deserves consistent check-ins, follow-up, and support. The Nurse Chronic Care Coordinator, Remote, will conduct telephonic encounters with patients monthly on behalf of Esrun Health's partners and develop detailed care plans within their electronic health record system. This role begins as an Independent 1099 Contractor position with the potential to transition to full-time W2 employment, including employee benefits. Esrun Health seeks Nurses to work part-time from a HIPAA-compliant home office, offering flexibility to set their own hours without a daily work hour schedule, though a minimum of 20 hours per week is contracted. Part-time team members typically dedicate 20-30 hours per week. Care Coordinators are assigned a patient panel (minimum 100 patients/month) and are expected to complete encounters for 90% of assigned patients. Esrun Health utilizes a productivity-based pay structure, with rates increasing based on the number of completed patient encounters per month. A patient encounter is defined as a minimum of 20 minutes, including chart review, call times/attempts/texts, care plan development, care coordination, and documentation time. Esrun Health, a division of Harris Computer, provides customized remote care services including Chronic Care Management (CCM), Remote Therapeutic Monitoring (RTM), Remote Physiologic Monitoring (RPM), Behavioral Health Integration (BHI), and/or Transitional Care Management (TCM). As a Harris healthcare business, it offers a small company experience with the financial security of a large organization, providing mission-critical software solutions globally.

Requirements

  • Graduate from an accredited School of Nursing. (LPN, LVN, RN, BSN, etc.)
  • Current license to practice as an RN/ LVN/LPN with no disciplinary actions noted.
  • A minimum of two (2) years of clinical experience in a Med/Surg, Case Management, and/or home health care.
  • Hands-on experience with Electronic Medical Records.
  • Understanding of Windows desktop and applications (Microsoft Office 365, Teams, Excel, etc.).
  • Being in a HIPAA compliant area in home to conduct Chronic Care Management duties.
  • Ability to exercise initiative, judgment, organization, time-management, problem-solving, and decision-making skills.
  • Skilled in using various computer programs.
  • High Speed Internet and Desktop or Laptop computer (Has to be operation system of Windows 10 or higher or Mac) NO Chromebooks and no iPad.
  • Excellent verbal, written and listening skills.

Responsibilities

  • Perform telephonic encounters with patients on behalf of our partners each month.
  • Develop detailed care plans within our care plan templates in the electronic health record.
  • Abide by the plan of care and orders of the practice.
  • Provide prevention and intervention for multiple disease conditions through motivational coaching.
  • Develop a positive interaction with patients on behalf of our practices.
  • Improve revenue by creating billable CCM episodes, increasing visits for management of chronic conditions.
  • Develops detailed care plans for both the doctors and patients for prevention and intervention purposes.
  • Understand health care goals associated with chronic disease management provided by the practice.
  • Attend regularly scheduled meetings (i.e., Bi-Monthly Staff Meetings, monthly one on one's, etc.).
  • Carry a patient panel of a minimum of 100 patients per calendar month.
  • Complete encounters on 90 percent of the patients they are assigned.
