About The Position

Under the direction, the CCHIP Clinic Team Coordinator (Coordinator) will be responsible for ensuring the teams' vision and PCMH standards are implemented; and that team roles and expectations are known and clear. The Coordinator will ensure that PCMH scopes are met through the monitoring of HEDIS measures and works with the clinic team to improve on these measures. This position will supervise CCHIP HE/CHW (Health Educator/Community Health Worker) with the aim to increase the number of patients engaged in care and successfully obtain and maintain viral load suppression. He/she monitors HIV health outcomes and provides reports to the Executive Team and the Clinic Operations Committee as needed. Facilitates pre-clinic huddles and discusses patients' needs to be addressed during the clinic session. Provides clinic coverage as needed or the other Coordinator and Specialty Clinic Coordinator. The Coordinator will be responsible for monitoring the PCMH clinic sessions, the patients empaneled to the sessions, and facilitate effective communications with the multi-disciplinary team members. Communicates with the multi-disciplinary through the message center on patient-related specific issues. Addresses patient needs as identified and connects the patients with the correct discipline as needed. Works with the Patient Access Manager to ensure the medical team clinical note documentation are closed timely. Participates and works with the CQI Committee as an active member related to PCMH measures and reports. The Coordinator will motivate, provide effective guidance, communicate with the multi-disciplinary team members daily, and be the contact person for all team members.

Requirements

  • Bachelor's degree from an accredited college or university in a health-related field, such as nursing, health administration, or social services, is required. (Must provide official transcript at time of interview)
  • One (1) year of experience working with the HIV population is required.
  • Minimum of 5 years proven work experience as a team leader is required.
  • Knowledge of performance metrics such as PDSA cycles is required.
  • Excellent verbal, written communication, and interpersonal skills necessary to communicate with all levels of staff and a patient population composed of diverse cultures and age groups
  • Knowledge and proficiency with Microsoft applications and internet-based programs
  • Strong interpersonal skills with the ability to establish strong working relationships
  • Ability to anticipate, recognize, and meet the needs of patients and families
  • Ability to work in a team-based environment to accomplish goals and objectives
  • Ability to demonstrate respect and sensitivity for cultural diversity in CCH work force and patient population
  • Ability to critically think, problem solve and make independent decisions supporting the authorization process, including interactions with payer representative, physicians, and hospital case managers
  • Ability to work with sensitivity to cultural, ethnic, and sexual diversity.

Responsibilities

  • Develops the clinic teams' goals based on PCMH requirements, expectations, and effective communication
  • Responsible for the overall coordination of PCMH sessions and ensures patients are linked to care and obtain viral load suppression
  • Facilitates effective communication between patients, team members, disciplines, and administrators
  • Monitors team and clinic performance using data metrics such as HEDIS measures and participates in the CQI Committee continuous quality improvement efforts
  • Encourages staff development and team building efforts
  • Oversees the teams' coordination of internal and external referrals and monitor processes
  • Adheres to Department's Documentation Policy of all Patient Encounters and Forms.

  • Participates in all CORE Center and department mandatory trainings and meetings.
  • Provides Excellent Customer Service and Professionals when interacting with clients and internal and external customers.
