We are currently looking to fill a CCBHC Triage Specialist role at our Merrionette Park office.

Requirements

  • Good computer skills and ability and willingness to learn a variety of applications.
  • Proficiency with Microsoft Office programs.
  • Must be detailed oriented with good organizational skills.
  • Excellent communication skills (verbal and written).
  • Excellent interpersonal skills.
  • Ability to multitask within a fast-paced environment and meet multiple deadlines simultaneously.
  • Demonstrated courtesy, patience, and good judgment under pressure.
  • Ability to work effectively with diverse individuals and groups.
  • Master’s degree in social work or a related behavioral science field. from an accredited college or university required .
  • Licensed Social Worker (LSW) or Licensed Professional Counselor (LPC); IL Qualified Mental Health Provider (QMHP) Required
  • Minimum 2 years of experience in behavioral health, crisis intervention, or intake settings.
  • Must obtain IM+CANS Certification within 30 days of hire
  • Proficiency in using HFS electronic eligibility systems (MEDI/REV) and Electronic Health Records.

Nice To Haves

  • Bilingual (English/Spanish) preferred
  • Knowledge of ASAM criteria for substance use disorder triage preferred
  • Experience serving Veterans and their families preferred
  • Ability to complete required 40-hour domestic violence training, preferred

Responsibilities

  • Clinical Triage & Assessment: Conduct initial screenings and comprehensive diagnostic assessments (mental health and substance use) to determine the appropriate level of care.
  • IM+CANS Completion: Use the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS) to document clinical needs, risk factors, and strengths for all new clients.
  • Crisis Intervention: Provide immediate de-escalation, safety planning, and short-term crisis counseling for individuals in acute distress.
  • Treatment Planning: Facilitate the initial Person-Centered Treatment Plan in collaboration with the client and their family, ensuring alignment with CCBHC quality measures.
  • Care Coordination: Work with Peer Support Specialists, Case Managers, and Clinicians to provide "warm handoffs" to internal programs.
  • Documentation: Maintain timely and accurate electronic health records (EHR) in compliance with 42 CFR Part 2 and HIPAA standards.
  • Effectively work with diverse staff and service population.
  • Other duties as assigned.
