We are currently looking to fill a CCBHC Triage Coordinator role at our Merrionette Park office.

Requirements

  • Good computer skills and ability and willingness to learn a variety of applications.
  • Proficiency with Microsoft Office programs.
  • Must be detailed oriented with good organizational skills.
  • Excellent communication skills (verbal and written).
  • Excellent interpersonal skills.
  • Ability to multitask within a fast-paced environment and meet multiple deadlines simultaneously.
  • Demonstrated courtesy, patience, and good judgment under pressure.
  • Ability to work effectively with diverse individuals and groups.
  • Bachelor’s degree in social work, Counseling, Psychology, or a related human services field, required OR a Bachelor’s degree in any field plus two years of documented clinical experience in a mental health setting under Qualified Mental Health Provider (QMHP) supervision, required
  • Must obtain IM+CANS Certification within 30 days of hire
  • Driving for work required with transportation of clients preferred in a personally owned vehicle.
  • Satisfactorily pass all relevant screenings and criminal background checks.

Nice To Haves

  • Experience serving Veterans and their families preferred
  • Proficiency in using HFS electronic eligibility systems (MEDI/REV) and Electronic Health Records.
  • Knowledge of ASAM criteria for substance use disorder triage preferred
  • Bilingual (English/Spanish) preferred.
  • Ability to complete required 40-hour domestic violence training, preferred

Responsibilities

  • Screening & Enrollment: Conduct initial screenings for mental health and substance use disorders using standardized tools as described in MFS CCBHC Policy and Procedures (e.g., PHQ-9, GAD-7, AUDIT-C).
  • IM+CANS Data Collection: Assist in gathering information for the Illinois Medicaid Comprehensive Assessment of Needs and Strengths (IM+CANS), focusing on social drivers of health and history, under the supervision of an LPHA.
  • Rapid Triage: Perform "front-desk" triage to identify individuals in acute crisis and immediately escalate to the on-site Crisis Team or Triage Specialist.
  • Benefits & Eligibility: Verify insurance/Medicaid status and assist unfunded clients with enrollment in available benefits to ensure no one is turned away regardless of ability to pay.
  • Waitlist & Schedule Management: Track and manage the "Time to Services" metric, ensuring routine evaluations are scheduled within 10 business days of first contact.
  • Resource Navigation: Provide "warm handoffs" and referrals as indicated for primary care, housing, vocational services, and other identified services and/or resources.
  • Documentation: Maintain timely and accurate electronic health records (EHR) in compliance with 42 CFR Part 2 and HIPAA standards.
  • Effectively work with diverse staff and service population.
  • Other duties as assigned.
