Summary The Child Center of NY has recently started a Certified Community Behavioral Health Clinic (CCBHC) through OMH. The CCBHC is to ensure access to coordinated comprehensive behavioral health care. All CCBHCs are required to serve anyone who requests care for mental health or substance use, regardless of their ability to pay, place of residence, or age. The Child Center of NY is seeking a compassionate and skilled Registered Nurse (RN) to join our team at our Certified Community Behavioral Health Clinic (CCBHC). The nurse will play a key role in ensuring and providing high-quality, patient-centered care to individuals experiencing mental health and substance use disorders. This position will involve collaborating with an interdisciplinary team to promote wellness, administer treatments, and provide education on behavioral health conditions. This position may involve supervising other nursing staff.

Requirements

  • Graduate of an accredited nursing program (Associate's or Bachelor's degree in Nursing).
  • Active RN license in the state of New York
  • Language other than English: Spanish
  • Training and experience working with people of all ages in behavioral health, mental health, or substance use treatment settings
  • CPR/BLS certification required.
  • Strong communication, problem-solving, and teamwork abilities
  • Ability to work amongst diverse social, cultural, and economic groups
  • Excellent time management, phone, and face-to-face engagement skills
  • Proficient in computer skills

Responsibilities

  • Assess clients need of a physical exam and conduct health assessments, including, but not limited to, communicable diseases, tobacco use, vital signs, medication reconciliation, and screenings for medical and behavioral health conditions
  • Coordinate medical care, including providing referrals to community medical providers, scheduling appointments, and liaising with external providers to obtain patients' records
  • Conduct metabolic monitoring (i.e. blood pressure, weight and body mass index)
  • Obtain and ensure processing of lab work for patients drawn both internally and externally
  • Administer medications, including injectable psychotropic medications, following prescribed guidelines and safety protocols
  • Provide basic first aid and manage medical emergencies and crisis situations appropriately
  • Monitor and document patient responses to treatment and medications, reporting concerns to the prescribing provider and treatment team
  • Provide patient education/counseling, both individually and in groups, regarding medications, chronic conditions, behavioral health diagnoses healthy eating, exercise and fitness, family planning, safe sex practices, harm reduction strategies for substance use, and general wellness
  • Engage in outreach and follow-up to promote patient engagement and adherence to treatment plans
  • Support crisis intervention efforts, including de-escalation and collaboration with emergency services when necessary
  • Coordinate with pharmacies to ensure patients have timely access to prescribed treatments
  • Maintain accurate and timely documentation in the electronic health record (EHR)
