About The Position

Provide consumers who have mental illness a quality coordination program, focusing on the individual’s strengths and desires. Maintain key working relationships with Eyerly Ball and UPH programs, Iowa Primary Care Association, Disability Access Points, other providers in the service network, consumers and families of consumers.

Requirements

  • Bachelor’s Degree required. Degree in Human Services or related field preferred.
  • One year of Mental Health experience required.
  • Valid driver’s license and insurance when driving any vehicle for work-related reasons.
  • Basic computer skills.
  • Knowledge of effects of mental illness and substance use.
  • Ability to speak, read and write fluent English.
  • Ability to read and comprehend common terminology of the mental health field.
  • Ability to enter documentation in an EHR and use Microsoft Office.
  • Ability to effectively present information and respond to questions from clients, medical professionals, management, staff and the public.

Responsibilities

  • Assess the needs of clients and develop goals that will allow them to achieve the greatest level of independence in their community.
  • Regular collaboration with client, treatment team and concerned others.
  • Advocate for clients to help meet their needs, desires and best interests.
  • Utilize critical thinking in making independent judgements and maintain responsibility and accountability for service provided.
  • Relate respectfully to clients with a wide range of abilities and diagnoses.
  • Meet clients in their homes or in public places with ability to drive safely, and a willingness to transport clients in own vehicle.
  • Provide in person case management and community-based services in all CCBHC counties.
  • Possess teamwork skills including problem solving, cooperation, negotiating, etc.
  • Continue professional development and involvement in professional activities that enhance service provision.
  • Complete all documentation within established guidelines.
  • Prioritize client need and complete required written documentation.
  • Demonstrate an understanding of all grant standards.
  • Track and document program quality measures.
