About The Position

St. Augustine Youth Services is seeking a Community Action Team (CAT) Case Manager to support children with serious emotional disturbances and their families in a community-based setting. This role focuses on coordinating care, advocating for services, and helping families stabilize and succeed. You’ll be working directly with a small caseload (1–3 children), allowing for meaningful, hands-on impact.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Counseling, Criminal Justice, Nursing, Human Services, Special Education, or a related field
  • 1 year of experience working with children with serious emotional disturbances OR 3 years of relevant experience with a bachelor’s degree
  • Knowledge of community resources for children and families
  • Understanding of state and federal regulations impacting behavioral health services
  • Strong communication and teamwork skills
  • Ability to work independently in a community-based environment
  • Professional presence when working with families, schools, and providers
  • Ability to manage sensitive situations and support youth in crisis

Responsibilities

  • Provide the five core functions of case management: assessment, planning, linking, advocacy, monitoring, and discharge planning
  • Complete initial assessments and develop service plans in compliance with Florida Medicaid guidelines
  • Work closely with youth and families to implement and adjust service plans
  • Coordinate care with treatment teams, schools, and community providers
  • Attend required meetings including MDT, IPT, school meetings, and staffing
  • Advocate for clients to ensure access to needed services and supports
  • Monitor progress and adjust services to meet client needs
  • Provide crisis support and intervention when necessary
  • Maintain accurate and timely documentation in compliance with Medicaid standards
  • Manage client records, reports, and electronic health records
  • Facilitate discharge planning and aftercare services
  • Participate in treatment planning and staffing with multidisciplinary professionals
  • Maintain and utilize a network of community resources
  • Provide transportation as needed to support service delivery
  • Conduct Wraparound meetings every 60–90 days
  • Maintain 80% compliance with documentation and service standards
  • Achieve 90% success rate in linking clients to services at discharge
