About The Position

Provides care management support services for consumers undergoing protective service investigations. Assists senior caseworkers by conducting reassessment visits, sets up in-home services, performs service-related errands and completes necessary case documentation for all service activities. REPORTING RELATIONSHIPS This employee reports to and has frequent contact with supervisor concerning care management issues and work assignments. Supervisory meetings are held monthly to review workload, and to discuss specific assignments, issues and concerns, and employee matters. Supervisor oversees work by obtaining feedback from senior caseworkers and reviewing the timeliness, completeness and accuracy of case entries, and case records in SAMS database. ESSENTIAL JOB FUNCTIONS Follows direction of senior caseworkers in establishing service for protective service consumers requiring in-home services and supports. Manages consumer cases. Completes comprehensive Individualized Service Plans (based on medical model and assessed need), documents according to requirements, interprets program policies and monitors consumer situations and provision of services on regular basis via home visits and phone contacts with consumers and caregivers to insure that services meet the needs of the consumer. Ensures a smooth and successful transition to other care management service (OPTIONS/CSP/Community Health Choices). (40%). Performs support service on behalf of the senior caseworkers including reassessment visits with consumer prior to case closure or transfer, conducts consumer errands and other related tasks at the direction of senior caseworker or supervisor. (25%). Completes required paperwork and case documentation. Utilizes quality assurance reports to ensure all case documentation is complete and accurate (35%). In the course of performing job duties, employee will be required on a routine basis to provide own transportation to consumers homes, meetings and other job-related appointments and access those locations without special accommodations. OTHER SPECIFIC TASKS OR DUTIES Maintains accurate knowledge of all aging services and thorough working knowledge of community services. Attends all mandated training sponsored by Department of Aging relative to community services and programs (including enrichment sessions). Performs other duties as assigned. Requirements

  • Successful completion of six (6) months of experience with the County as a Caseworker Trainee; or
  • Successful completion of the County Social Caseworker Intern Program; or
  • One (1) year of experience in public or private social work and a bachelor’s degree which includes or is supplemented by 12 semester hrs credit in sociology, social welfare, psychology, gerontology or other related social sciences: or
  • A bachelor’s degree with a social welfare major; or
  • Any equivalent combination of experiences and training including successful completion of 12 semester hrs credit in sociology, social welfare, psychology, gerontology or other related social sciences.
  • Current knowledge of HIPAA regulations and their interpretation is maintained and adhered to.
  • Give and receive information and feedback in order to provide a more complete result.
  • Demonstrate respect for and awareness of others, good judgment in responding to conflict, and flexibility to adapt to changes in one's work environment.
  • Understand and anticipate customer needs.
  • Acceptable Pre-Employment Criminal Background Check

  • Acceptable Driver History in accordance with County Policy
  • Must successfully complete the Pennsylvania Department of Aging Protective Services Basic training by the end of the first year of employment and annual refresher trainings, thereafter.
