Job Description: JOB SUMMARY Perform initial intake, screening, and problem identification of inquiries from those seeking information/assistance or assessments and referrals for home and community-based services. REPORTING RELATIONSHIPS Reports to and consults daily with Information and referral (I&R) Supervisor who monitors work via observation of interactions, reviewing of referrals, and thoroughness of case documentation/data entry. Attendance at I&R and Care Management unit meetings also occurs wherein worker receives regular updates on program policies and procedures. ESSENTIAL JOB FUNCTIONS Receive inquiries (in person, phone, or email) from older adults, families, potential referral sources and public at large. Conduct problem identification by screening and thoroughly exploring inquires to determine most appropriate response to the need(s) presented. Provide information/assistance about agency and community resources or programs requested and conduct eligibility determinations accordingly. Take/make referrals for appropriate programs and services such as level of care assessments and screening for Protective Service Reports of Need, demonstrating ability to receive/process such abuse reports in a timely, knowledgeable and professional manner. Follow-up as needed ensuring that all required documentation/data entry is thoroughly and clearly completed. 50% Assist with completing applications for Lottery-funded and other special programs such as Property Tax/Rent Rebate, PACE, LIHEAP, Healthy Horizons, etc. 25% Maintain updated community resource information for access by and distribution to agency units and general public. Participate in various public education opportunities, expos and community speaking engagements by representing the agency and providing information on available programs/services. 25% (Actual percent of time spent on these essential duties may vary with the needs of the agency.) OTHER SPECIFIC TASKS OR DUTIES Maintain records of contacts with consumers as needed. Attend relevant and/or required trainings as specified. Serve as advocate for older consumers. Perform other related duties as required.

Requirements

  • Successful completion of six (6) months of experience with the County as a Caseworker Trainee; or
  • One (1) year of experience in public or private social work and a bachelor's degree which includes or is supplemented by 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences; or a bachelor's degree with a social welfare major; or
  • A bachelor's degree with a social welfare major; or
  • Any equivalent combination of experience and training including successful completion of 12 semester hours credit in sociology, social welfare, psychology, gerontology, or other related social sciences.
  • Acceptable Pre-Employment Criminal Background Check
  • Acceptable Pre-Employment Drug Screen
  • Acceptable Driver History in accordance with County Policy
