About The Position

The Caseworker works within the Clinical Department, with the Aging and Community Living Supports (ACLS) programs. The caseworker conducts in-home ongoing assessments, and phone consultations to assist in identifying participant needs and arranging services and supports to meet these needs which, facilitates program participants living with dignity and independence in the community. The Caseworker coordinates services provides ongoing follow-up to program participants and completes various tasks as needed to support team member functions.

Requirements

  • Bachelors Degree in the Human Services field and 1-2 years of experience minimum.
  • Community-based position, must have independent transportation, and a valid driver's license.
  • Excellent communication, organization, interpersonal, problem-solving, and time management skills desired.
  • Strong computer and data entry skills in Word, Excel and Outlook required.

Nice To Haves

  • Preferred knowledge of agency programs, senior services, Medicaid long-term care programs, and local community resources.
  • Ability to effectively use or learn the EMR system for community referrals and program intake

Responsibilities

  • Conducting ongoing in-home reassessments and phone consultations with participants and/or their representatives, reviewing changes in areas such as health, physical function, environment, finances, cognition/mood, and informal support.
  • Identifies needs and completes follow-up with participants and/or community agencies for ongoing service coordination.
  • Answer calls from participants and/or their representatives and resolve the callers’ needs/provide follow-up, as needed.
  • Complete other tasks as assigned to support the programs, including ensuring that program forms are completed by participants, placing service requests for new participants, and assisting in the training of new hires.
  • Provide backup to other Caseworkers, as needed, or to the Program Coordinator with tasks such as processing mail, faxes, scanning, and attaching documents to participant records.
  • Ensure compliance and adherence to ACLS service standards, and meeting quality work metrics.
  • Conducting ongoing in-home reassessments and phone consultations with participants and/or their representatives, reviewing changes in areas such as health, physical function, environment, finances, cognition/mood, and informal support.
  • Average caseload: 80
  • Other duties as assigned
