The Case Worker is responsible for conducting interviews with clients to assess their needs and challenges, such as personal and family adjustments, financial difficulties, employment issues, housing concerns, and physical or mental impairments. They gather information on medical, psychological, and social factors affecting the client's situation and evaluate their capabilities. The Case Worker provides counseling and support to help clients develop plans to meet their needs, mobilize internal and external resources, and improve their social functioning. They monitor client progress, refer them to community resources, maintain accurate documentation, and ensure effective communication with supervisors and funding sources. Additionally, the Case Worker may assist with housing individuals and providing necessary supplies and support to help them maintain their housing stability. This Position is Full time at 40 Hours a Week, Monday to Friday 8:30 pm to 5:30 pm. Pay For This Position is $23.50 Per Hour.

Requirements

  • Associate Degree in Human Services or a related field
  • Three years of experience working with vulnerable individuals
  • Equivalent combination of education and experience will be considered
  • Valid N.Y.S. Drivers License
  • Registered and insured vehicle
  • N.Y.S. Child Abuse Registry clearance
  • Mobility sufficient to drive to and participate in meetings at client's homes and/or community facilities
  • Some locations may not meet Federal accessibility standards for disabled individuals
