About The Position

Provides comprehensive, structured, case management services for emergency shelter residents. Learns and adapts to the uniqueness of a resident’s history to determine the most effective relationship approach. Develops comprehensive program plan/goals and evaluates residents’ progress through one-on-one sessions and diagnostic tools. Advocates for community resources and services that will enable residents to resolve or ameliorate the underlying causes of their homelessness.

Requirements

  • Associate degree in social work, human services, or related field from an accredited college or university; AND One year experience providing direct case management social services including accessing residents’ needs and developing individual, comprehensive, long-term action plans for recovery utilizing a wide variety of resources; OR Any equivalent combination of training and experience providing the required knowledge, skills, and abilities.
  • Ability to meet attendance requirements.
  • Ability to read, write, and communicate the English language.
  • Ability to communicate clearly and effectively both orally and in writing.
  • Ability to work on multiple cases effectively and efficiently without becoming frustrated or disorganized.
  • Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.)
  • Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking.

Responsibilities

  • Conducts resident intake and an initial needs assessment at the time of entry to the shelter.
  • Provides linens, toiletries, and hygiene products; replenished as needed throughout residents’ stay.
  • Meets individually with residents to discuss and evaluate their progress, feelings, impressions, changes, and personal growth/development; prepares accurate and up-to-date records documenting the same.
  • Prepares and maintains case records and logs; ensures the accuracy and completeness of the same; enters pertinent information into the Homeward Community Information System (HCIS).
  • Refers residents to additional counseling resources if needed.
  • Maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.
