About The Position

Under general supervision, the Case Worker Health Educator uses advanced skills gained through training and experience to provide specialized educational counseling services within an assigned functional area. Regularly communicates information to clients regarding program services. Follows established procedures to perform routine tasks and receives general guidance and direction to perform other work with substantial variety and complexity requiring limited decision-making responsibility. Originality and ingenuity are required to locate, select, and apply appropriate procedures, processes and techniques to assignments that are broad in nature. Resolve most questions and problems and may serve as a resource to others in resolving complex problems and issues. Routine contact with internal and external customers is required to obtain, clarify, or provide facts and information.

Requirements

  • Knowledge of Community resources including social services agencies and crisis intervention providers
  • Processes, rules and regulations of various services and funding agencies surrounding the eligibility and maintenance of the services being received
  • Basic health and wellness, fitness principles, as well as healthy living behaviors is required
  • Basic EMR functionality
  • Communicate effectively, both orally and in writing
  • Instructing others on how to do a task
  • Operating standard office equipment and using required software applications for program area and other applications, including Microsoft Office
  • Partner with other functional areas to accomplish objectives
  • Organize and prioritize multiple tasks and meet deadlines
  • Communicate effectively, both orally and in writing
  • Speak and write in English.
  • Ability to interpret, speak and write in Arabic, Spanish or other native language of the predominant service population is preferred
  • Maintain confidentiality of agency and client information
  • Associate degree in Business, Marketing, Public Health, Social Work or Human Services based on assigned functional area
  • At least two years of experience in sales, marketing, public health, social work, or human services.

Nice To Haves

  • Some experience in group facilitation in person and using a video conferencing platform as well as experience in motivational interviewing and lifestyle change health coaching is preferred, or any equivalent combination of experience, education and/or training approved by Human Resources
  • If employed in the diabetes program a c ertification as Diabetes Prevention Lifestyle Coach or obtained within 3 months of hire

Responsibilities

  • Provide advocacy, culturally appropriate health education and information regarding assigned program objectives to clients and communities
  • Providing care coordination, case management, and system navigation activities (assuring access to care)
  • Building individual and community capacity (teaching those served how to manage their own health needs)
  • Implementing individual and community assessments to determine needs and/or identify risks
  • Conducting outreach (home visiting to provide education, assessment, and social support)
  • Document all services in compliance with the agencys quality assurance plan
  • Report on program progress at required intervals per funding guidelines
  • Create educational/program marketing materials targeted toward assigned program including newsletters, Facebook and websites
  • Participate in analytical analysis of the program and identification of participant characteristics
  • Organize activities and/or fundraising events in support of program objectives
  • Maintain agency confidentiality and clients privacy
  • Maintain currency regarding services provided to clients
  • Follow the funding sources requirements and CARF standards applicable to the services provided
  • Facilitates delivery of ACCESS Diabetes Prevention Program, provides support, service (e.g., taking blood pressure, diabetic foot care, etc) and guidance to program participants, and maintains fidelity to the curriculum
  • Provide a cultural bridge among individuals, communities, and health and social service systems (act as bridge between providers and patients)
  • May travel to community locations for program delivery and/or deliver program virtually
  • May attend training events, meetings and conferences as needed to fulfill funding requirements
  • May translate and interpret information for non-English speaking clients as needed
  • May conduct presentations to audiences of various sizes and demographics
  • May assist and enroll clients in programs, services and benefits offered and referring clients in need of other services outside the scope of the area
  • Operate standard office equipment and use required software applications
  • Perform other duties and responsibilities as assigned
