About The Position

The University of Miami/UHealth Department of Pediatrics has an exciting opportunity for a full time Case Worker to work in Miami, FL. This direct service position is responsible for providing maternal child case management services and community outreach to Healthy Start clients (pregnant women and infants) participating in the Florida Healthy Start Program. Responsibilities include Healthy Start care coordination/case management services including the following: client initial assessments and enrollment, comprehensive case management/care coordination, psychosocial referral services, and wraparound services including parenting education, breastfeeding support, tobacco cessation, etc. Must be willing to conduct regular home visits with personal vehicle to provide services to clients and interact with other community providers. Position is salaried, and required working evenings and weekends as needed to ensure that job duties are met. Responsibilities also include completion of all required paper and electronic documentation on time and in compliance with all programmatic and contractual requirements. CORE JOB FUNCTIONS Conducts patient evaluations to obtain information on the patient's medical and mental health, treatment and recovery to develop an effective treatment plan. Ensures patients are enrolled in insurance and free or low-cost social service programs for which they are eligible (e.g., food stamps, WIC, housing, SSI, Medicaid/Medically Needy Program, Ryan White, and ADAP, etc.). Learns the requirements of the new Affordable Care Act and the · enrollment process to ensure patients know how to appropriately enroll or transition their care from Ryan White to a health care exchange or other insurance program that will allow them to maintain their care with our program. Identifies and resolve real or perceived barriers to care, (e.g., disclosure, cultural beliefs, denial, transportation, childcare, support systems, functionality, insurance, etc.) to enable patients to adhere to care recommendations and improve their health outcomes. Assists with patient-provider communication. Provides medication adherence education, assessment, and counseling. Conducts secondary prevention and lifestyle management interventions. Provides clients with information on available clinical trial opportunities and facilitates linkages to research staff. Educates and counsels patients around issues related to their illness and/or life situation and assists in finding additional information from websites and agencies. Maintains knowledge of community health and welfare resources and the variety of programs offered. Maintains confidentiality of all records, data, and information as required by HIPAA regulations and other regulatory agencies Documents filing and actions taken, according to departmental guidelines and standards. Assists with record audits for documentation and record compliance. Adheres to University and unit-level policies and procedures and safeguards University assets.

Requirements

  • Bachelor’s degree in relevant field required
  • Minimum 1 year of relevant experience required
  • Fluent in Spanish or Creole
  • Learning Agility: Ability to learn new procedures, technologies, and protocols, and adapt to changing priorities and work demands.
  • Teamwork: Ability to work collaboratively with others and contribute to a team environment.
  • Technical Proficiency: Skilled in using office software, technology, and relevant computer applications.
  • Communication: Strong and clear written and verbal communication skills for interacting with colleagues and stakeholders.
  • Any relevant education, certifications and/or work experience may be considered.

Responsibilities

  • Provide maternal child health services to CONNECT clients, including: client initial intakes.
  • Work in conjunction with supervisor and multidisciplinary service team to ensure implementation of coordinated services as related to program participant needs.
  • Provide case management/care coordination referrals to participants when appropriate for the range of maternal child health psychosocial and behavioral health needs.
  • Perform quality assurance tasks, service-related administrative duties and data tasks related to the preparation of programmatic documentation and agency reports and compliance with program performance measures.
  • Function as a contributing member of the team during staff meetings and with other team members.
  • Conduct Healthy Start educational outreach activities in community settings and facilitate collaborative relationships with community partners.
  • Complete all required client related service and programmatic written and electronic documentation in compliance with programmatic requirements, agency reporting, and the Healthy Start Standards and Guidelines.
  • Attend all programmatically required trainings and staff meetings and strictly adhere to program protocols.
  • Other organizational/support duties as assigned.
