Under the supervision of the Specialized Health and Social Services (SHSS) program supervisor, the Case Manager provides complex and comprehensive public health social work and case management services for children with special health care needs and their families, as well as high-risk pregnant women seeking services from local, state, and federal programs. The role includes conducting assessments, providing information and referral services, and training program contractors. The Case Manager works under minimum supervision with considerable latitude for initiative and independent judgment, completing administrative duties and participating in disaster response efforts as needed.

  • Conduct home visits for comprehensive assessment of home environment, psychosocial, economic, and special medical needs of the child and family.
  • Provide case management services for families in various health programs.
  • Conduct assessments and reassessments for Personal Care Services/Community First Choice.
  • Participate in community organization functions and collaborate with other entities.
  • Attend regional and program staff meetings and trainings.
  • Complete administrative duties, monthly reports, and special assignments.
  • Provide consultation, presentations, and training to service providers.
  • Current license to practice Social Work in the State of Texas.
  • Undergraduate or Graduate Social Work degree from an accredited university.
  • Knowledge of case management principles and practices.
  • Skill in using computer software for word processing, spreadsheets, and databases.
  • Ability to work effectively as a team member and maintain confidentiality.
  • Experience with community resources on a local, state, and national level.
  • Familiarity with cultural and socioeconomic issues related to health and disabilities.
