About The Position

We are seeking a compassionate and dedicated Case Manager to provide comprehensive services to adults with severe and persistent mental illness (SPMI). This position is an integral part of an inter-disciplinary team focused on promoting recovery, wellness, and goal attainment. The Case Manager will work directly with clients to provide community-based supports, including resource development, crisis intervention, treatment planning, and promoting good health.

Responsibilities

• Provide case management services to support clients in managing severe and persistent mental illness.
• Develop and connect clients to resources that aid in their recovery, such as housing, healthcare, and employment.
• Promote and encourage healthy lifestyle choices, focusing on both physical and mental health.
• Offer crisis intervention in emergencies to de-escalate situations and ensure client safety.
• Work collaboratively with the interdisciplinary team to create and implement individualized treatment plans to meet specific client goals.
• Monitor client progress and adjust treatment plans as necessary to ensure successful goal achievement.
