About The Position

Our case management team is designed with specialized expertise so we can meet clients where they are and provide the most effective, compassionate care possible. Each Case Manager brings a focused skill set to support the unique needs of the individuals and families we serve. The Case Manager is responsible for providing comprehensive resources, advocacy, and direct support to a diverse population in need of assistance with substance use, medical or behavioral health concerns, food insecurity, and other social determinants of health. This position requires strong collaboration with internal programs and external community-based organizations to ensure clients receive the full spectrum of resources and services available. We are currently seeking: Human Trafficking Case Manager – to walk alongside survivors of trafficking, offering advocacy, safety planning, and connections to community resources. Substance Use Disorder Case Manager – to support clients navigating recovery by providing guidance, encouragement, and access to treatment services. Early Childhood and Families Case Manager (Ages 0–5) – to partner with caregivers and young children using the Parents as Teachers model, strengthening family stability and supporting healthy child development. This structure allows our team to provide trauma-informed, individualized care—ensuring that every client receives the specialized attention and resources they need to move forward.

Requirements

  • Deep understanding of trauma-informed care principles and best practices in supporting survivors of violence
  • Proven ability to manage complex cases and navigate multiple service systems effectively. Demonstrating leadership and initiative
  • Strong computer skills and proficiency in case documentation; adept in Microsoft Word, Outlook and other relevant software
  • Excellent judgment and problem-solving skills to navigate complicated situations while collaborating with a diverse team of professionals
  • Experience working with culturally diverse populations and a desire to implement best practice in addressing their needs to bring about equitable outcomes. Respect for other’s experiences, opinions, language, culture and knowledge
  • Comprehensive knowledge of community resources and demonstrated ability to ensure effective service coordination
  • Valid California driver’s license and access to reliable transportation
  • Strong communication skills, both within a team setting and with clients
  • Time management skills are essential

Nice To Haves

  • BA or BS degree in social work, human services, psychology, or related field preferred; or a minimum of three years’ experience working with survivors of domestic violence, sexual assault, and/or human trafficking
  • Basic knowledge and ability to work in computer programs (i.e. Word, Excel, etc.)
  • Utilizes Trauma Informed Care and understanding of ACES when working with clients
  • Satisfactory completion of 66-hour domestic violence and sexual assault intervention training (post-hiring requirement) and ongoing completion of continuing education hours to maintain state certification as a domestic violence and sexual assault advocate/counselor
  • Ability to pass a background investigation including fingerprinting

Responsibilities

  • Complete thorough Client Needs Assessments following initial Client Services intake
  • Perform ongoing assessments, safety planning, and creation of a Care Management plan
  • Actively promote client empowerment and self-advocacy
  • Provide education and support as needed, including education around Domestic Violence, Sexual Assault, Parenting, Substance Use, etc.
  • Maintain thorough and accurate case notes and documentation to track client progress
  • Complete initial engagement documentation and meet monthly client billing requirements
  • Ensure all informed consents are signed and protocols for information sharing are adhered to
  • Provide intensive case management including referrals to services, and regular check in with clients and assisting with transportation to ensure attendance for important appointments
  • Present case information at case staffing meetings to ensure coordinated care
  • Participate in shelter case staffing and attend weekly leadership meetings to assess client service and operational goals
  • Other duties as assigned
  • Provide accurate verbal and written translation and interpretation between English and Spanish to support client communication and documentation needs
