In this role, you will perform a wide variety of patient care activities under the guidance of the Executive Clinical Director. Collaborating with the Treatment Team, the Case Manager supports the master treatment plan and coordinates discharge planning. They maintain communication with referral sources, facilitate access to additional services as needed, and provide essential support to the Clinical Team.
