About The Position
The Case Manager at Sierra Health and Wellness is responsible for providing direct client services in a field-based setting, working collaboratively with other case managers, therapists, counselors, and community providers. The role focuses on facilitating client wellness and autonomy through advocacy, assessment, planning, and resource management, ensuring that care is safe, effective, and client-centered.
