About The Position

The Case Manager at The Jewish Board plays a crucial role in supporting older youth transitioning to independent living through the Supervised Independent Living Program (SILP). This position involves providing counseling, case planning, and resources to youth aged 18-21, helping them develop the necessary skills for independent living while ensuring they receive the support they need during this critical transition.

Requirements

  • A bachelor's degree is required; a master's degree in human services or a related field is preferred.
  • Willingness to work with young adults facing trauma and mental health challenges.
  • Compassion and respect for vulnerable individuals.
  • Patience and understanding for the stabilization and recovery process.

Nice To Haves

  • Familiarity with Connections (ACS record) is preferred.
  • Experience with telehealth services.

Responsibilities

  • Carry a caseload of clients and provide services within the scope of program regulations and agency mission.
  • Evaluate and assess independent living skills and support clients in meeting individual goals.
  • Teach clients self-advocacy skills by modeling advocacy techniques.
  • Link clients to community-based treatment and collaborate with service providers for coordinated services.
  • Practice person-centered and collaborative service planning with clients, ensuring service plans are completed and signed according to regulatory guidelines.
  • Interpret and explain agency policies and procedures to clients.
  • Run groups under the supervision of the clinical supervisor.
  • Support residents in achieving medication management independence through appropriate interventions.
  • Accompany residents to appointments as needed.
  • Make weekly home visits during business hours and after hours as necessary.
  • Attend court for clients when required.
  • Participate in intake, assessment, discharge, and follow-up conferences with service providers and ACS/OCFS as needed.
  • Participate in regular staff meetings and training sessions.
  • File incident reports and complete necessary paperwork in a timely manner, including case notes and court reports.
