About The Position

The Case Manager position involves providing comprehensive case management services to clients with developmental disabilities. The role focuses on conducting assessments, developing individualized service plans, and advocating for clients by connecting them with necessary resources. The Case Manager will also provide crisis intervention, collaborate with other professionals, and maintain accurate case records, all while working independently in a hybrid environment.

Requirements

• Minimum of 2 years of experience in case management or related field
• Strong knowledge of social work principles and practices
• Experience working with diverse populations, including individuals with behavioral health needs, child welfare involvement, or workforce development needs
• Familiarity with programs such as Section 8, Medicare, and childcare assistance
• Ability to provide mentoring and support to clients in a compassionate and non-judgmental manner
• Excellent communication and interpersonal skills
• Strong crisis management skills

Responsibilities

• Provide comprehensive case management services to clients with developmental disabilities
