Requirements

  • High school diploma or GED equivalent
  • Valid driver's license
  • CADC certification within two years of employment with TASC may be required based on contractual obligations
  • Must be able to pass an IDOC Background check
  • Demonstrated proficiency in the Microsoft Office suite
  • Demonstrated experience in group facilitation
  • Knowledge of the current ASAM placement criteria, DSM-V as it relates to substance-related disorders, and Code of Ethics for substance abuse professionals, or the ability to acquire the knowledge
  • Comfort working inside a correctional setting and with persons convicted of serious violent offenses
  • Flexibility in thinking and approaches to leading group discussions
  • Good verbal, written, and interpersonal communication skills
  • Good time management skills with a proven ability to meet deadlines

Responsibilities

  • Conduct comprehensive assessments and develop individualized service plans
  • Facilitate intake, enrollment, and coordination of services inside correctional facilities and in the community
  • Provide referrals and crisis intervention, including behavioral health, housing, public benefits, and employment
  • Collaborate with internal teams and external partners to ensure holistic and integrated care
  • Monitor and document client progress toward service goals, using systems like ASAM criteria and DSM-5
  • Complete accurate, timely records including progress notes, service plans, and discharge summaries
  • Assist clients in obtaining vital records and enrolling in public benefit systems
  • Attend team meetings, staffings, and community partner events to support client care
