About The Position

The Recovery Support Advocate at Elwyn plays a crucial role in providing therapeutic support and clinical interventions to individuals ranging from young adults to geriatric clients facing mental health challenges. This position emphasizes person-centered, strength-based approaches to promote recovery and stability in a therapeutic setting or within the community. The advocate coordinates care with various partners and ensures that service delivery is integrated and recovery-oriented, while also maintaining accurate records and documentation.

Requirements

  • Bachelor's degree in Human Service field, such as Social Work, Psychology, Rehabilitative Counseling, or closely related discipline required.
  • Five or more years of behavioral health direct care experience can be substituted for a Bachelor's degree.
  • One year full-time experience in providing direct care service to mentally ill consumers preferred.
  • Preference will be given to multi-lingual candidates who meet the cultural and linguistic needs of the population served.

Responsibilities

  • Coordinates care with MA Behavioral Health Community Partners, One Care, and DMH Case Managers to facilitate optimum service delivery.
  • Understands the signs and symptoms of mental illnesses and intervenes therapeutically in a recovery-based environment.
  • Empowers individuals through advocacy and skill building.
  • Exercises good judgment and decision-making, particularly during crisis situations.
  • Assures that support/care promotes relationship building and is based on clinical best practices.
  • Demonstrates knowledge of prescribed medications and follows state-specific medication guidelines.
  • Maintains individuals' records in accordance with agency policy and procedures.
  • Produces timely, accurate, and thorough recordkeeping in accordance with agency, state, or federal regulations.
  • Demonstrates competency in the development, implementation, and modification of treatment plans based on comprehensive client assessments.
  • Provides psycho-educational services to persons served and their families as required.
  • Supports community integration and linkages to community resources.
  • Integrates the PRISMModel® for effective treatment of serious and persistent mental illness into everyday practice and clinical intervention.
  • Encourages ongoing engagement and involvement of family members in all phases of service delivery and evaluation.
  • Develops communication plans with family members as agreed upon with individuals.
  • Performs other duties as required.
