The Case Manager is responsible for engaging assessing, planning, and coordinating services for high school students and/or their families, ensuring they receive the necessary support to achieve their academic goals. This position involves working with students from various backgrounds, often those facing complex life challenges, to ensure that their needs are met in a holistic and coordinated manner. The Case Manager will also monitor progress and advocate for clients to achieve the best possible outcomes.

  • Conduct thorough assessments of students’ needs, including health, social, financial, and emotional factors.
  • Create and implement individualized care or service plans based on student assessments.
  • Connect students with relevant in-school and community-based resources to support their academic goals.
  • Monitor students’ progress toward their goals, adjusting the care plan as necessary.
  • Advocate on behalf of clients with external agencies, healthcare providers, or other organizations.
  • Maintain accurate and up-to-date records of client interactions, assessments, care plans, and outcomes.
  • Provide immediate assistance in crises, such as medical emergencies, housing instability, or other urgent client needs.
  • Work with a multidisciplinary team to ensure coordinated care and comprehensive support for clients.
  • Educate clients on their rights, available services, and strategies for managing their health and well-being.
  • Regularly evaluate the effectiveness of the case plan and make adjustments as needed.
  • A bachelor’s degree in human resources, Social Work, Business Administration, or a related field (or equivalent experience).
  • Minimum of 2 years of experience in job development, employment counseling, human resources, or a related field.
  • Strong communication and interpersonal skills, including the ability to build rapport with clients from diverse backgrounds.
  • Excellent organizational and time-management abilities.
  • Problem-solving and critical thinking skills.
  • Ability to navigate complex systems and advocate for clients effectively.
  • Knowledge of community resources and services.
  • Proficiency in Microsoft Office and case management software.
  • Experience working with high school-aged youth, such as individuals with disabilities, veterans, or marginalized groups.
  • Familiarity with community resources and support services.
  • Advanced certification in case management.
