About The Position

As a Case Manager in the Keeping Kids in School program at Seneca Family of Agencies, you will play a crucial role in supporting youth and families in Sonoma County. Your primary focus will be to help remove barriers to school attendance and engagement, empowering students to thrive through personalized support, skill-building, and resource connection. This position requires an active, onsite presence within partner districts and the community, allowing you to make a meaningful impact in the lives of K-12 students.

Requirements

  • Bachelor's degree OR Associate's degree or equivalent + 1 year of applicable work experience OR High school diploma/GED + 2 years of applicable work experience.
  • Applicable experience includes any paid or unpaid experience working with youth or families, or working in the social services field.
  • Own vehicle, valid California driver's license, and insurability with Seneca's automobile policy.
  • Must drive daily within Sonoma County as part of the role.
  • Expected to be on-site at the assigned school campus and in the surrounding community 4 to 5 days per week.
  • Must be at least 21 years of age.
  • Clearance of TB test, fingerprints and any other mandatory state/federal requirements.
  • Proof of COVID-19 vaccination.

Nice To Haves

  • Bilingual in Spanish and English
  • Two or more years of direct work experience in community or school-based services youth and families
  • Strong organizational skills as demonstrated by the ability to effectively manage multiple tasks.

Responsibilities

  • Conduct assessments to identify needs, barriers, and strengths to establish individualized goals and action plans with clients, caregivers, and school teams.
  • Facilitate monthly action plan review meetings with clients, families, school teams, and other participating members.
  • Provide youth and families with linkage to school and community resources to meet their needs and establish sustainable solutions.
  • Integrate into assigned school communities by working with administration and school intervention teams, attending multidisciplinary meetings, and participating in other school events.
  • Develop and maintain healthy partnerships with a variety of mental health, school, and community providers.
  • Provide technical assistance in the form of school or district wide interventions and participating in school or district wide events.
  • Advocate for creative problem solving to address underlying student/family barriers to success in school.
  • Make efforts to understand the history of Black, Indigenous, People of Color (BIPOC) in the United States and further Diversity, Equity, and Inclusion.
