About The Position

The Case Manager at Palms Medical Group is responsible for providing exceptional customer service and ensuring the proper coordination of patient referrals. This role involves managing patient data, processing referrals, and maintaining communication with both patients and healthcare providers to facilitate seamless care. The Case Manager plays a crucial role in supporting the healthcare team and enhancing patient experiences within the organization.

Requirements

• High School Diploma or GED.
• 1 year of prior experience in insurance verification and referrals.
• Working knowledge of Microsoft Office Suite, including Word and Excel.
• Ability to operate office equipment such as multiline telephones, copiers, and scanners.

• Experience in a healthcare setting.
• Familiarity with Electronic Health Records (EHR).
• Strong organizational and multitasking skills.

Responsibilities

• Provide first-class customer service to patients of Palms Medical Group.
• Answer, screen, and route telephone calls in a polite and professional manner.
• Document phone calls in the Electronic Health Record (EHR).
• Process referrals according to established policies and workflows.
• Ensure complete demographic and insurance information is sent to referral specialists.
• Explain the referral process to patients, including steps and contact information for specialists.
• Track referrals and maintain communication with patients and specialists regarding their status.
• Contact insurance companies to ensure prior approval requirements are met.
• Maintain patient confidentiality in accordance with HIPAA regulations.
• Assist with audits and surveys as directed by management.
