About the position

The Case Manager is responsible for overseeing case management and coordinating various services for guardianship clients living in the community. This role involves advocating for clients' rights, monitoring their medical appointments, and ensuring they receive necessary entitlements. The Case Manager collaborates with public guardians and treatment teams, maintains client documentation, and responds to emergencies, all while promoting the safety and well-being of clients.

Responsibilities

  • Tailor services based on the individual abilities/disabilities of clients
  • Advocate for individual rights
  • Monitor and assist with medical appointments and activities
  • Provide transportation to client appointments using agency or personal vehicle
  • Ensure client's entitlements are secured (e.g. SSI, SSDI, DHS, Medicaid, Bridge Card)
  • Collaborate with the Public Guardian and treatment team to share relevant information about clients
  • Communicate with client's family members, public guardian, and providers, maintaining appropriate documentation
  • Attend supervision, trainings, staff meetings, and medication reviews
  • Manage client files and annual updates
  • Respond to client emergencies during and after office hours
  • Document all contacts and information relating to the client
  • Perform any reasonable activity that promotes the safety, security, well-being, and best interest of the client
  • Complete other duties as assigned.

Requirements

  • Minimum two years college or an Associates Degree in a Human Services field
  • Three years responsible experience in a related Human Services field
  • Bachelor's Degree in a Human Services field preferred
  • Ability to communicate with various social agencies and represent the client's interest
  • Ability to understand medical issues
  • Ability to make decisions regarding the client's well-being
  • Proficient computer skills in Microsoft Office Suite and Outlook
  • Ability to research and problem solve independently
  • Knowledge of County operations and functions
  • Ability to prioritize and manage time effectively
  • Ability to handle stressful situations and provide effective customer service
  • Ability to think critically when necessary
  • Knowledge of basic administrative tasks and office equipment operation
  • Ability to communicate effectively with management, co-workers, and clients
  • Knowledge of complex accounts receivable/payable practices and banking procedures
  • Proficient in accounting software, i.e. Bi-Tech or One Solution
  • Knowledge of accounting principles, audit standards, budgeting, and IRS practices.

  • Experience in supervising
  • Knowledge of liability coverage and claims procedures.
