The primary responsibility of the Case Manager is the overall development and implementation of the student’s Performance Plan. Case Managers should be acutely aware of the Dove Vocational Trauma Informed Practices and engage both staff and youth through the lens of trauma. Essential responsibilities and duties include, but are not limited to: Essential Functions: 1. Monitors and documents the student’s overall progress while in vocational, educational, work and leisure settings. 2. Conducts intake and program orientation procedures with each new student in accordance to program policy. 3. Conducts Treatment Team Meetings with each student on a Bi–Weekly basis. 4. Complete all phases of the Ray for each student within the required time frames. 5. Completes a Needs Assessment for each new student within the required time frame. 6. Participates in the student’s Exit Conference to ensure all preparations for student departure have been made. 7. Effectively addresses student unacceptable behavior through verbal dialog and documentation. 8. Completes an individualized Performance Plan for each student in accordance with program policy. 9. Document on-going revision to student Performance Plans. 10. Develops a written Transition Plan for each student departing within the required time frames. 11. Interacts with students appropriately while in vocational, educational, work and leisure settings. 12. Prevents and intervenes in student altercations and incidents. 14. Effectively communicates student information and concerns with other employees. 15. Assist in establishing a Community Action Team (CAT) to ensure a smooth transition into the community. 16. Will drive Dove / Twin Oaks vehicles as required in accordance with the DJJ Policy. 17. If needed, will provide emergency First Aid and CPR in accordance with American Red Cross Standards. 18. Conducts and practices emergency evacuation procedures. 19. Attends and participates in all required meetings and trainings. 20. Is actively involved with assisting each student meet their goals while in the program. 21. Adheres to all program policy and procedures. 22. Assumes other duties and responsibilities as directed or needed for efficient operations of the program. 23. Implement Impact of Crime/Restorative Justice Curriculum via group facilitation. 24. Attend and participate in Clinical Supervision with DMHA at least 1 hour per week face to face.
