Outreach is hiring a Full-time Case Manager at Outreach Carol Stream to reduce housing instability and support long-term resident success for residents in the Carol Stream community in the Autumn Ridge Apartments and Villagebrook Apartments. Requirements

Bachelor’s degree in social work, education, or a related field
At least three to five years of direct service experience preferred
At least one year of administrative experience
Demonstrated ability in program administration and demonstrated sensitivity to the needs of individuals from low-income and culturally diverse backgrounds
A current, valid driver’s license is required along with a good driving record
A commitment to Outreach’s Statement of Christian Mission and Faith

Responsibilities

The Case Manager will support the recertification process
conduct benefits eligibility screenings
connect residents to employment and financial stability resources
link residents to services available through the Outreach Carol Stream community center
connect residents in rent or utility delinquency to community and emergency resources
coordinate responses to lease or eviction-related concerns
provide targeted advocacy and crisis-oriented case management
actively participate in the Outreach Carol Stream Case Management Team for routine support and staffing
providing client transportation assistance as needed
