About The Position
Centurion Forensic Health Services partners with the Connecticut Judicial Branch to provide community-based forensic and mental health services to the Court Support Services Division Alternative in the Community (AIC) programs. The company is seeking a full-time Case Manager for its Derby Alternative in the Community office in Derby, Connecticut. The Case Manager is responsible for the assessment, planning, and coordination of transitional services for offenders, including their re-integration from correctional facilities to the community. This role involves ensuring clinical services or clinical placement upon release to promote continuity of care.
Requirements
- Bachelor’s degree from an accredited university or college; degree may be substituted with experience on a year-for-year basis
- Two (2) years’ experience administering assessments and performing case management functions
- Must be able to pass background investigation and obtain agency security clearance
