About The Position

Case Manager – Superior Futures for Youth Lutheran Social Services of Wisconsin and Upper Michigan (LSS) is seeking a dedicated Case Manager to support youth and young adults ages 12–24 experiencing homelessness through our Superior Futures for Youth program. In addition to youth services, this role includes 10 hours per week supporting the Rapid Re‑Housing (RRH) program, a light‑touch model that assists adults and families experiencing literal homelessness in securing permanent housing and maintaining stability through short‑term rental assistance. This is a full‑time, benefit‑eligible position offering the opportunity to make a meaningful impact in the lives of vulnerable young people and households experiencing housing instability.

Requirements

  • Bachelor’s degree in social work, sociology, psychology, or a related field or an equivalent combination of education and experience (subject to COA/contract requirements).
  • Minimum of two years of professional experience .
  • Strong knowledge of counseling and casework principles, social service systems, community resources, and relevant regulations.
  • Understanding of human behavior, group dynamics, and social issues.
  • Ability to provide professional social work services, manage time effectively, build relationships, and prepare detailed reports.
  • Valid driver’s license, reliable transportation, and ability to meet LSS driving and insurance requirements.
  • Ability to travel throughout the service area up to 75% of the time; occasional overnight travel may be required.

Nice To Haves

  • State certification such as CSW or CPC is preferred.

Responsibilities

  • Conduct assessments, develop case plans, and provide ongoing case management for youth ages 12–24.
  • Coordinate community, social, and mental health services to meet client needs.
  • Maintain accurate case records, prepare required reports, and participate in hearings as needed.
  • Build strong relationships with clients, partners, and community agencies.
  • Attend trainings, staff meetings, and community events; represent LSS professionally.
  • Support agency quality initiatives and perform additional duties as assigned.
  • Manage a small caseload of up to six households experiencing literal homelessness.
  • Meet with clients, document services, and assist with securing permanent housing within 60 days.
  • Support employment‑related goals and promote long‑term housing stability.
  • Attend county meetings related to RRH programming.
  • Assist with housing searches, applications, and landlord communication.
  • Submit monthly rent checks and coordinate rental assistance for eligible clients.
  • Provide resources and referrals to community partners for needs outside RRH’s light‑touch scope.
  • Maintain communication with landlords and partners to support client stability.
