Case Manager - Inpatient Child/Adolescent Unit
About The Position
Hartford HealthCare, a comprehensive healthcare network with over 40,000 colleagues, is seeking a Case Manager for Natchaug Hospital. Natchaug Hospital is Eastern Connecticut’s leading provider for children, adolescents, and adults dealing with mental illness and substance abuse addictions. The hospital offers a variety of programs including inpatient and outpatient behavioral health, residential treatment for court-involved teenage girls, geriatric services, clinical day treatment schools, and chemical dependency services. The Case Manager will be responsible for providing a range of services to clients with psychiatric and/or substance abuse issues, offering advocacy, outreach, and service linkage, and initiating access to entitlement services.
Requirements
- Experience working with a behavioral health and/or chemical dependency population
- Bachelor's degree in Human Services or related field
- Valid driver’s license may be required based on program
- Computer proficiency and keyboarding skills required
Nice To Haves
- Social Work degree preferred
- Knowledge of community resources preferred
Responsibilities
- Providing a range of services for clients with either Psychiatric and/or Substance Abuse issues
