Requirements

  • Bachelor’s degree preferred or High School Diploma/GED with 1 year Case Management experience working with formally homeless individuals.
  • Experience using Access HRA and securing HRA entitlements and benefits preferred.
  • Strong oral and written communication, time management and organizational skills are necessary.
  • Proficiency in Microsoft Office suite and other standard business technology is required.

Responsibilities

  • Support families in maintaining permanent housing through regular one on one meetings which may include home visits and ongoing engagement.
  • Assess, identify and address early signs of housing instability to prevent eviction and housing loss.
  • Provide direct services to clients, including intake, engagement, assessment, service plan development, referral, advocacy, counseling, exit summaries and follow-up.
  • Monitor and document client progress toward service plan goals.
  • Document all interactions, timely, with, or on behalf of, clients in respective database.
  • Maintain familiarity with program resources available on- and off-site.
  • Coordinate client services with other social service, mental health, medical, employment, educational, childcare and other providers.
  • Complete program reports on a monthly basis, or more frequently as required.
  • Act as program contact person for inquiries concerning clients.
  • Provide collaboration and support with securing benefits, especially as it relates to access to benefits services and eviction prevention efforts.
  • Report to, and meet with, supervisor on a monthly basis.
  • Assist with community events as needed and serve as a liaison with community vendors.
  • Perform additional administrative duties as assigned.
