About The Position

The PATH (Projects for Assistance in Transition from Homelessness) Case Manager provides direct outreach, engagement, assessment, and case management services to individuals experiencing homelessness who have serious mental illness, co-occurring substance use disorders, or other disabling conditions. This role focuses on helping clients access housing, behavioral health care, benefits, and other resources, while ensuring accurate and timely data collection in HMIS in compliance with Data Standards and SAMHSA PATH Program requirements.

Requirements

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field required.
  • Minimum 3 years’ experience providing case management, outreach, or supportive services to individuals experiencing homelessness or with serious mental illness and/or substance use disorders.
  • Strong interpersonal and relationship-building skills with diverse populations.
  • Proficiency in assessment, service planning, and crisis intervention.
  • Ability to navigate complex service systems and advocate effectively for clients.
  • Competence in HMIS data entry, accuracy, and timeliness.
  • Understanding of trauma-informed care, harm reduction, and Housing First principles.
  • Ability to work independently and as part of a multidisciplinary team.
  • Valid driver’s license and reliable transportation required.
  • Bachelor’s degree from an accredited program in a helping field.
  • Must possess a valid GA Driver’s license.

Nice To Haves

  • Experience working in street outreach or housing-focused programs preferred.
  • Knowledge of HUD HMIS data entry and reporting processes preferred.
  • SOAR certification preferred or ability to obtain within 6 months.

Responsibilities

  • Conduct street outreach, shelter visits, and community-based engagement to identify and connect with individuals who may be eligible for PATH services.
  • Build trust and rapport with clients through consistent, person-centered, and trauma-informed interactions.
  • Complete assessments to determine PATH eligibility, needs, and appropriate service referrals.
  • Provide case management services, including developing individualized service plans, linking clients to housing, mental health, substance use treatment, medical care, and mainstream benefits.
  • Deliver and document PATH-funded services and PATH-funded referrals in HMIS accurately and on time.
  • Record all contacts, Date of Engagement, PATH Status, and SOAR connections according to PATH data collection workflow.
  • Advocate for clients with service providers, landlords, and benefit agencies to reduce barriers to housing and care.
  • Assist clients with applications for benefits, including connecting them to SOAR-trained staff when appropriate for SSI/SSDI applications.
  • Maintain regular contact with clients to support goal achievement, housing retention, and linkage to ongoing care.
  • Participate in case conferencing, team meetings, and training to maintain program compliance and strengthen service delivery.
  • Maintain professional boundaries and confidentiality in accordance with HIPAA and agency policy.
