This role includes identifying achievable goals and objectives, developing programs within the Therap electronic records system, and monitoring implementation for quality and compliance. The Case Manager works closely with Direct Support Professionals and supervisors to ensure health, safety, and meaningful programming. This position also coordinates vocational opportunities and ensures both vocational and non-vocational needs are being met. What You’ll Do Develop and implement Individual Service Plans and structured goal programs Enter and maintain programming within the Therap system Monitor goal implementation and provide periodic written updates Provide leadership and direction to Direct Support Professionals Ensure overall health, safety, and well-being of assigned participants Coordinate vocational opportunities with Community Supports Supervisor Maintain accurate documentation including activity, progress, incident, attendance, and statistical reports Meet with participants, families, and outside agencies as needed Participate in team meetings and program planning Provide crisis management and behavioral intervention when necessary Assist participants with daily living activities as needed Take rotating on-call responsibilities to cover staffing and scheduling needs Clearly communicate Parc’s mission and values within the community This role requires both administrative expertise and hands-on engagement What We’re Looking For
