The Critical Time Intervention (CTI) program is a 9-month intense intervention program designed for individuals 18 years or older with complex mental health conditions. The focus is to support during critical transitions in care, including those with previous inpatient admissions or long inpatient stays requiring enhanced follow-up care and links to outpatient support; those frequently accessing emergency rooms, comprehensive psychiatric emergency programs and other crisis services; and those at risk of losing housing. The program ensures strength based, person-centered quality of care provided in a culturally and linguistically meaningful therapeutic environment. The case manager is an integral part of the program, coordinating support and services for the identified clients.
