About The Position

The Critical Time Intervention (CTI) program is a 9-month intense intervention program designed for individuals 18 years or older with complex mental health conditions. The focus is to support during critical transitions in care, including those with previous inpatient admissions or long inpatient stays requiring enhanced follow-up care and links to outpatient support; those frequently accessing emergency rooms, comprehensive psychiatric emergency programs and other crisis services; and those at risk of losing housing. The program ensures strength based, person-centered quality of care provided in a culturally and linguistically meaningful therapeutic environment. The case manager is an integral part of the program, coordinating support and services for the identified clients. Requirements

  • Bachelor’s degree in human services ( i.e. psychology; social work; etc).
  • A year of experience as a case manager, preferably field-based work
  • Ability to use business acumen to exercise good judgment and decision-making.
  • Excellent interpersonal skills and able to communicate both verbally and in written form.
  • All other duties as assigned by the Director and Assistant Director.
  • On Call Responsibilities as designated by the Director and Assistant Director

Responsibilities

  • Provide direct coaching, education, escort, and advocacy in linking, engaging, and retaining clients in services identified in the Plan of Care.
  • Conduct vigorous outreach in identifying and locating potential clients either referred through the community or by the lead CTI.
  • Provide intensive care management services to clients to obtain the full range of needed services and ensures coordination of these services.
  • Conduct outreach in accordance with Critical Time Intervention policy via phone, letter, and field work to client/collateral/provider/ support system to engage clients or strengthen connectivity.
  • Conduct home visits to clients as needed to provide comprehensive care management services.
  • Complete the plan of care and ensure all comprehensive assessments are finalized in a timely manner.
  • Complete and document productivity of a minimum of five services per day.
  • Responsible for the overall chart compliance of assigned caseload members.
  • Participate in quality improvement activities, projects, and reviews.
  • Identify new sources of potential clients and conduct outreach presentations as requested.
  • Meet regularly with supervisor and attend staff meetings. Be prepared to discuss clinical and operational issues impacting performance and program operations.
  • Maintain and update caseload tracking tool.
  • Utilize company issued cell phone to stay in contact with members/ providers/ team on a 24-hour basis.
  • This position requires extensive fieldwork, with 85–90% of time spent providing direct, in-person services and client engagement within the community.
