About The Position

A Case Manager is the main point of contact for all residents to present any concerns, recommendations or grievances. All interactions shall be conducted with a holistic approach. Case Manager will be a liaison and involve all family, staff and M.D. as needed an is responsible for thorough follow up. This position is also an intricate part of the admissions process. The primary job responsibilities for this position include, but are not limited to, the following: Mail Sorting/Handling Filing Reporting Interacting with Government agencies · SS · DOH · OTDA Medicaid · Applications · Re-certifications · Billing issues · Forms Understanding of financial documents · Create, read, and explain basic financial analysis · Accounts receivable – assist in resolving and explaining Referrals – Placements Referring Elderplan or other Medicare Advantage plans Explaining/assisting with special forms · POA, HCP, MOLST, DNR Wills, Living Wills, Pre-paid burial plans Discharges Compliance Issues Various Other Tasks

Requirements

  • Must be responsible, consistent and able to meet commitments

  • Must be computer literate
  • Knowledge of Microsoft Office Suite a plus. (Excel, Access, Word etc)
  • Knowledge of Google Suite
  • Knowledge of an EMAR system helpful
  • Ability to learn new software as needed
  • Excellent phone skills
  • Must have Valid NYS Driver’s license
  • Must have at least three years experience in a HR or Service Delivery position, such as Case Management or Social Work or
  • a Bachelors or Master’s Degree with a minimum of one year work experience in a dependent adult population setting.
  • Must have current (within 30 days of employment) Physical.
  • Must have current PPD test with a negative result.

Nice To Haves

  • Knowledge of Microsoft Office Suite a plus. (Excel, Access, Word etc)
  • Knowledge of an EMAR system helpful
