About The Position

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of different programs throughout Arizona. CBI is the premiere non-profit fully integrated healthcare provider of substance use and behavioral health programs in Arizona, including prevention, education and treatment using cutting-edge, nationally recognized, evidence-based models. Job Summary The Case Manager provides case management services in either the community and/or office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching and advocating, coordination and monitoring the patient's needs and progress, to assist patients in gaining access to health services, financial assistance, housing, employment, education, social services and natural supports. The Case Manager assesses patients needs, develop and monitor individualized treatment plans and document services provided in an accurate and timely manner. The Case Manager embraces the recovery philosophy and promotes a safe recovery environment for clients. The Case Manager will be responsible for implementing advanced techniques and interventions to serve the high-risk individuals in their associated program.

Requirements

Highschool diploma or GED required.
1-3 years of experience working with the GMH/SU population required.
1-3 years of experience in behavioral health position providing outreach and engagement activities required.
Current AZ Driver's License (valid and in good standing preferred).
Valid ID required
Arizona Fingerprint Clearance card
Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures

Nice To Haves

Associates Degree or higher in a field related to behavioral health preferred.
1-3 years of experience in medical setting preferred.
Minimum of six months of recovery from substance use and/or mental health disorders preferred.
Or equivalent combination of education and experience
Clean 39-month driving record (MVR) preferred.
